Diagnostic Imaging requests

Visit the Monash Health website for information on Imaging services.

A request form can also be found on the Monash Health services page under Referrals. This form can be used to request ultrasounds for morphology, fetal growth and wellbeing through Monash Imaging. Medicare-eligible women will be bulk billed.
